Sensory Integration Data Collection Form

Description

This form was designed to assist educational staff to use when working one on one with students during trials in order to measure if and how often sensory integration is needed to complete the designated prompt. Staff would need to determine the approximate time intervals, write in the activity being completed, note the time on task and whether or not sensory integration was needed. Staff would also need to note the time of sensory integration implemented and whether or not it assisted the students in completing the designated task.
