This form should be sent to teachers after students with problematic behaviors are sent out of class in order to collect qualitative and quantitative data. Collected and synthesized data assists IEP teams in developing relevant and accurate IEP and FBA/BIP goals. Collected data can also negate the hypothesized IEP, FBA, and/or BIP goal(s) an IEP team will need to reconvene in order to make the necessary adjustments. The step ladder referred to is the emotional recognition lesson which provides a visual for psychological triggers. The goal is to reduce the frequency of student removal from class with the consistent use of behavioral supports and interventions. This can be used in any grade level and in the total school environment.
