This form should be sent to teachers after students are removed or sent out of class
• The purpose of this form is to assist in the qualitative and quantitative data collection
• Collected and synthesized data assists in the accurate calculation of IEP and FBA/BIP goal
• Should the data negate the hypothesized IEP, FBA, and/or BIP goal(s) an IEP team will
need to reconvene in order to make the necessary adjustments
• Goals must be written in measurable terminology and data collected consistently in order
to determine the effectiveness of each goal.
